This notice describes how health and medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. The privacy of your health and medical information is important to us. This Notice takes effect 12/08/2025 and will remain in effect until we replace it.

Our Responsibilities

We understand that medical information about you and your health is personal. Applicable federal and state law requires us to maintain the privacy of your health information and to give you this Notice about our privacy practices, our legal duties, and your rights concerning your health information. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We reserve the right to change our privacy practices and this Notice at any time, as permitted by law, and will make the updated Notice available upon request.

Uses and Disclosures of Health Information

We may use and disclose health information about you for treatment, payment, and healthcare operations, including:

• To Treat You — sharing information with providers involved in your care
• Billing and Payment for Services
• Healthcare Operations — quality assessment, training, credentialing, and related activities
• Your Authorization — with your written consent, for any purpose you specify (revocable at any time)
• To Your Family and Friends — only with your agreement, to help with your care or payment
• Persons Involved in Care — to notify or assist family/representatives in emergencies, using professional judgment
• Marketing — we will not use your health information for marketing without your written permission
• Required by Law — including compliance reviews by Health and Human Services Abuse, Neglect, or
• Safety Threats — disclosed to appropriate authorities when necessary
• National Security — limited disclosures involving military, intelligence, or correctional authorities as required
• Organ/Tissue Donation and Medical Examiner/Funeral Director requests
• Workers’ Compensation, Law Enforcement, and Government Requests Lawsuits and Legal Actions — in response to a court order or subpoena
• Appointment Reminders — via voicemail, postcard, text, or letter

Your Rights

• Access — request to view or receive copies of your health information (a written request and reasonable cost-based fee may apply)
• Disclosure Accounting — a list of certain disclosures made over the last 6 years (not before April 14, 2003)
• Restriction — request additional limits on use/disclosure (not guaranteed, but honored once agreed to)
• Alternative Communication — request contact by alternate means or locations (written request required)
• Amendment — request corrections to your records (written request with explanation required)
• Records Transfer — your records transfer with a practice sale/merger; you may request they go elsewhere instead
• Electronic Notice — if received online or by email, you’re entitled to a written copy on request
